When you are ready to apply to colleges, you must request a transcript to be sent to each school.
Transcript & School Report Request Form - please complete this Google form (see link below) when you are ready to apply to colleges. Please keep deadlines in mind, and give the guidance office at least 5 days notice to prepare the report and transcript. If a recommendation is required from your school counselor, please click on the form below, complete and turn it in to Guidance giving at least 5 days to complete as well.
SAT Scores on Transcript: The transcript request form will ask if you want to have your ACT/SAT scores on your transcript. The decision applies to all transcripts and can not be changed depending on the college.
